Maternal and Child Health (MCH) services are a priority program in health development because they are related to efforts to reduce the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). However, MCH services at the community health center level still face various financing challenges, such as budget constraints and cost planning that is not fully based on actual service needs. Therefore, a unit cost analysis is needed to determine the amount of service costs and assess the suitability of financing with the operational needs of MCH services. This study aims to identify direct and indirect cost components, calculate unit costs, and compare the actual cost of services with Health Operational Assistance (BOK) funds for MCH Cluster services at the Antang Community Health Center in Makassar City.
This study used a quantitative descriptive design with a top-down costing approach. Data were obtained through financial report documentation, program activity reports, and KIA Cluster service data at the Antang Community Health Center in 2025. The analysis was conducted on out-of-home family planning services, immunizations, prenatal classes, toddler classes, and visits by high-risk pregnant women (resti).
The results of the study indicate that transportation costs for officers are the largest direct cost component in most services. Immunization services have the highest total cost of Rp149,402,801, while the toddler class has the lowest total cost of Rp6,018,247. The highest unit cost is found in visits to high-risk pregnant women (resti) at Rp135,801 per service, while the lowest unit cost is found in immunization services at Rp14,786 per service. Comparison with BOK funds shows that immunization services and resti visits experience a funding deficit, while other services experience a budget surplus.
The study's conclusions indicate that the unit cost of KIA Cluster services is influenced by the characteristics and volume of services. Services with high target coverage have lower unit costs than home visit-based services. Furthermore, BOK funds serve as operational support and do not fully cover the costs of all KIA services.
