Wound healing is a complex biological process that can be influenced by therapeutic agents. Moringa oleifera leaf extract, as a natural therapeutic agent, contains secondary metabolites with the potential to promote wound healing. This study aimed to determine the effectiveness of Moringa oleifera leaf extract gel on epithelial thickness during the healing process of incision wounds in Rattus norvegicus. The experimental study employed a post-test-only controlled group design using 24 Sprague–Dawley rats. The animals were divided into four treatment groups (n = 6): control (0.9% NaCl), 10% povidone iodine, 3% Moringa leaf extract gel (GEDK), and 9% GEDK. Epithelial thickness was assessed histopathologically on days 3, 7, and 14. Data were analyzed after meeting parametric assumptions, including normality and homogeneity tests, followed by one-way ANOVA and Tukey’s HSD post hoc tests. The results showed that GEDK at concentrations of 3% and 9% increased epithelial thickness in rat skin incision wounds over time, with the 3% concentration producing a statistically significant increase across observation periods. However, the mean epithelial thickness at both concentrations of Moringa leaf extract gel remained lower than that of the 0.9% NaCl control group. Therefore, formulation optimization and further studies are required before this gel can be recommended as a primary wound care option. A limitation of this study is the duration of observation, which was limited to 14 days (the proliferation phase); consequently, epithelial thickness during the tissue maturation phase could not be comprehensively evaluated.
